CAFS Golf Tournament Registration Form

• Cost is $230 per person (+5% GST), includes tailgate party, golf and cart, BBQ steak dinner, major draw prizes, cash gift and door prizes.
• Payment must be received by CAFS no later than May 31, 2018.
• Any unpaid registrations become open to those on the waiting list.
• Cancellations after June 1st are non-refundable, however names can be substituted.
